Nursing Transcript Request Form

Graduates from both campuses may request academic transcripts using the Practical Nursing Transcript Request Form.  Please mail the required $3 processing fee and the form to the Oil City campus at the following address:

Practical Nursing Program

Oil City, PA 16301

 

***Official transcipts can only be sent to Employers and/or Educational Institutions***

Please note that in order for an official transcript to be generated for a student, the student must complete the entire first phase of the program.
